Med Minder Program
The Med Minder program is designed to prevent medication errors and increase patient/doctor communication about medication. The card allows patients to keep a current, accurate list of all of their medications in one, easy-to-locate area. Rather than reading each individual bottle for instructions, the medications and their related dosages are listed in an easy to read format. The portable card can be brought to appointments to ensure doctors and pharmacists know all medications the patient is using, including over the counter drugs and vitamins.
Med Minder also provides space to record personal medical information, including allergies and emergency contact numbers. It has tips on keeping medications safe, and lists questions to ask your healthcare professional to better understand your medications.
