Two Nurse Practitioners Welcomed to BAHC

Nicole Schopf, APNP and Tammy Plondke, APNP
Boscobel Area Health Care welcomes Tammy Plondke, APNP, and Nicole Schopf, APNP, to their medical staff.
Plondke will provide primary care for patients at Riverside Family Practice in Muscoda, and Schopf will provide primary care for patients at Bluff Street Clinic in Boscobel. In addition, both will provide care for BAHC's Walk-In / Urgent Care center.
As nurse practitioners, both women can provide a variety of primary care services: Diagnose and treat illnesses; manage chronic diseases; prescribe and manage medications; perform well-child checks, sports physicals, and annual exams. Both women are qualified to care for a patient through his/her entire lifespan.
Plondke and Schopf both worked in nursing before pursuing their master's degrees in family practice nursing.
Schopf has worked at Meriter for eight years, serving in their neuroscience center and their intermediate care center. "I am looking forward to the transition from hospital care to preventive care. A primary goal is to help patients maintain their health and prevent hospitalizations," said Schopf. Annual primary care exams include early-screening services, such as blood pressure checks, cholesterol checks, pap smears, and other services proven to detect disease in early stages when conditions are most treatable.
Plondke has worked as a nurse at Prairie du Chien Memorial Hospital for 19 years. "I have worked every area - Intensive Care Unit (ICU), Emergency, Obstetrics, Med/Surg, and Pharmacy." Plondke chose to pursue her master's degree and become a nurse practitioner so she could help navigate patients through the healthcare system. "I wanted to work with patients one-on-one and develop a relationship with each patient. As a nurse, I saw people when they were at their sickest. As a nurse practitioner, I will be able to continue their care afterwards, helping them stay well and preventing further hospitalizations."

Wanek said BAHC is very proud of patient wait times in the ER and Walk-In Care Centers. "Our wait times continue to be well below national and state averages. Additional medical providers will allow us to continue to grow, without compromising our commitment to see patients in a timely manner." said Wanek.
